29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  This violation was observed on or about  March 13, 2013 at the construction site where employees were exposed to an approximate  34 foot fall hazard when performing roofing work without a fall protection system.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees engaged in residential construction activities are protected from falling when exposed to falls 6 feet or more above a lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   The employer does not ensure that ladder used by employees is extended 3 feet above the upper landing surface. This violation was observed on or about March 13, 2013 at the construction site where employees were exposed to an approximate 14 foot falll hazard when the ladder was used to gain access to roof .     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that ladders used by employees have the ladders side rails extended at least three feet above the upper landing surface and is secured its top to a rigid support.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  The employer does not ensure that ladders used by employees  were free of structural defects. This violation was observed on or about March 13, 2013 at the construction site where employees were exposed to an approximate 14 foot fall hazard when using a ladder that had bent rungs.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that ladders used by employees  are free from structural defects.
