29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. Job site located at 10708 S 149th Street, Omaha, NE 68138-The employer allowed employees to use a powered industrial truck where the forks of the lift had holes drilled in the tips of the fork that could affect the capacity and safe operation. The employer did not have prior written approval to have the holes in the tips of the forks.
