29 CFR 1904.32(a)(2): The employer failed to create an annual summary and then certify the annual summary of work-related injuries and illnesses for the previous years: Rehkemper and Son Inc.: On or about March 18, 2013 the employer did not record the following workplace injuries and illnesses on the OSHA 300 Log for the calendar year 2011. a) On or about February 24, 2011, a recordable injury occurred when an employee received medical treatment due to a fall that resulted in a broken leg from jumping off stacked trusses. b) On or about April 19, 2011, a recordable injury occurred when an employee received medical treatment due to a shoulder strain which occurred while using a hammer to build trusses. c) On or about October 4, 2011, a recordable injury occurred when an employee received medical treatment due to a laceration to their middle finger from a razor blade used to cut material.

29 CFR 1904.32(a)(2): The employer failed to create an annual summary and then certify the annual summary of work-related injuries and illnesses for the previous years: Rehkemper and Son Inc.: On or about March 18, 2013 the employer did not record the following workplace injuries and illnesses on the OSHA 300 Log for the calendar year 2012. a) On or about June 4, 2012, a recordable injury occurred when an employee received stitches due to a laceration on their hand from a movement against a metal flip plate. b) On or about June 6, 2012, a recordable injury occurred when an employee received medical treatment due to a laceration on their finger that occurred when their finger caught a plate on the trusses. c) On or about June 22, 2012, a recordable injury occurred when an employee received medical treatment due to shoulder soreness which occurred when the employee was stacking lumber. d) On or about July 18, 2012, a recordable injury occurred when an employee received medical treatment due smashed finger which occurred when the employee was using a hammer.
