29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale (dBA), or equivalently a dose of fifty percent:  The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equaled or exceeded an equivalent dose of fifty percent:  a)  On or about April 26th, 2019, an employee in the wall panel assembly area was exposed to noise at 106.4% of the permissible daily dose, or an equivalent average sound level of 90.3 dBA as measured over 486 minutes of sampling on a 9-hour shift.  The equivalent average sound level of 90.3 dBA is greater than the adjusted action level of 84.2 dBA for a 9-hour shift.   b)  On or about April 26th, 2019, an employee in the wall panel assembly area was exposed to noise at 90.5% of the permissible daily dose, or an equivalent average sound level of 89.2 dBA as measured over 480 minutes of sampling on a 9-hour shift.  The equivalent average sound level of 89.2 dBA is greater than the adjusted action level of 84.2 dBA for a 9-hour shift.   c)  On or about April 26th, 2019, an employee attending the Green Saw was exposed to noise at 87.9% of the permissible daily dose, or an equivalent average sound level of 88.2 dBA as measured over 535 minutes of sampling on a 9-hour shift.  The equivalent average sound level of 88.2 dBA is greater than the adjusted action level of 84.2 dBA for a 9-hour shift.  Note:  The data above is reflective of noise dosimetry using a criterion level of 90 dBA, a noise threshold of 80 dBA, a 5 dB exchange rate, and slow response.
