American Manufacturing Operations, Inc. located at 2375 Dorr St. Toledo, OH 43612; On or about March 20, 2013, the employer did not assure the employees who are exposed to the unexpected startup and/or release of stored energy of the Logan Hydraulic Press were properly trained in the skills required for the safe application, usage, methods, and means to isolate and control energy sources. Employees that accessed hazard areas of the machine for servicing, were not trained to apply a lockout device to the machine.

Employees are required to perform die changes on the Logan Press exposing them to amputation hazards. American Manufacturing Operations, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i) which was contained in OSHA inspection number 465178, citation number 1, item number 1 and was affirmed as a final order on December 12, 2012 with respect to a workplace located at 2375 Dorr St. Toledo, OH 43607.
