29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment were not isolated from the energy source and rendered inoperative:     (a) On or about 6/21/2013 at Solid Surface Craftsman, Inc., operators of and an employee doing maintenance on the laminator were exposed to electrical and caught-in hazards associated with unexpected energization because the employer failed to establish a lockout tagout program.      (b) On or prior to 9/17/2013 at Solid Surface Craftsman, Inc., an employee doing maintenance on the CombiCut machine was exposed to electrical and mechanical hazards associated with unexpected energization because the employer failed to establish a lockout tagout program.     (c) On or prior to 09/17/2013 at Solid Surface Craftsman, Inc., an employee doing maintenance on the bridge saw was exposure to electrical and mechanical hazards associated with unexpected energization because the employer failed to establish a lockout tagout program.
