29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board:    The employer did not provide a guardrail system, or equivalent, for employees, who walked and/or worked on top of the automatic pinsetters when servicing, maintaining, repairing and/or unjamming the machines.   This violation occurred on March 20, 2013, and at times prior thereto, the employees, who were clearing jams, servicing, repairing motor, were exposed to the hazard of falling into pinsetter machines.

This violation occurred on March 20, 2013, and at times prior thereto; when the employees, who were exposed to the hazard of falling more than 24-inches while servicing, repairing motor, unjamming the pinsetters onto which they had climbed without the benefit of fixed stairs or equivalent means of safe access.

The unguarded pinch points created by moving levers, cams, shafts, arms, cracks, rods and rake boards and springs of the pinsetter machines #1 through #48.    b.     The Elevator wheels of the pinsetter machines #1 through #48, were not guarded to prevent employees from reaching into the machines.

This was occurred on March 20, 2013, when employees, who serviced, maintained, repaired, and/or unjammed pinsetter machines #1, #4, #6, were exposed to an electrical hazard from the outer insulation of the power cords to the control boxes separating from the attachment plug cord clamp exposing the conductors to pull.
