29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about March 15, 2013, an employee was using a circular saw that was powered by an extension cord that did not have strain relief, exposing the employee to an electrical shock hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about March 15, 2013, the owner and employees were accessing the roof of the building under renovation with a 20 foot extension ladder that did not extend at least 3 feet above the landing, exposing the employees to a fall hazard.
