29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: a) On or about March 11, 2013, employees engaged in roofing activities on a nearly vertical sloped roof, while working off a ladder jack scaffold, 21 feet above the ground level, while wearing a Personal Fall Protection System harness, were not connected to a horizontal life line. There was only one lifeline for the two employees, and neither was connected to it.
