29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   Location:  780 Shackelford Road, Florissant, MO  63031  At the jobsite, six workers were observed exposed to fall hazards ranging from an approximate 8 foot fall hazard to an approximate 10 foot fall hazard as they performed residential roofing activities on an approximate 2:12 pitch roof without any type of fall protection system.    The Starrett Home Repair was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 460416, citation number 1, item number 1 and was affirmed as a final order on 07/26/2012, with respect to a workplace located at 2143 Grand Ave, Granite City, IL  62040.    The Starrett Home Improvement was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315785618, citation number 1, item number 1 and was affirmed as a final order on 01/25/2012, with respect to a workplace located at 42 Woodfork Maryland Heights, MO  63043.    The Ray Starrette DBA Big Dog Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 313017345, citation number 1, item number 2a and was affirmed as a final order on 01/05/2011, with respect to a workplace located at 1498 Vandalia Street, Collinsville, IL  62234.    The Ray Starrette DBA Big Dog Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313016917, citation number 1, item number 2a and was affirmed as a final order on 01/05/2011, with respect to a workplace located at 4102 Troy Road, Glen Carbon, IL  62034. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Location:  780 Shackelford Road, Florissant, MO  63031  The employers training program did not enable each employee to recognize the hazards of falling and was insufficient to train each employee in procedures to be followed in order to minimize fall hazards while employees worked on a 2:12 pitch roof which was 8-10 feet above the next lower level. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Location:  780 Shackelford Road, Florissant, MO  63031  Employees were using an extension ladder that did not extent at least 3 feet over the landing (roof) and were exposed to a fall hazard of approximately 10 feet, 6 inches.
