29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 254 W Palo Verde Circle, Sacaton, AZ: On or about 21 March 2013 and at times prior thereto, employees applying asphalt roof shingles to the residential construction were not protected against the fall hazard. The eave height of the residential construction was approximately 9 feet.

29 CFR 1926.95(d)(1): Except as provided by paragraphs (d)(2) through (d)(6) of this section, the protective equipment, including personal protective equipment (PPE), used to comply with this part, was not provided by the employer at no cost to employees: (a) 254 W Palo Verde Circle, Sacaton, AZ: On or about 21 March 2013 and at times prior thereto, employees were required to provide their own hard hat at the jobsite and use it protect them from head injury.
