29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training, or in lieu of the actual date of the training that prior training for the employee conducted by another employer was adequate and the signature of the employer: (a) Location: 245 W Palo Verde Circle, Sacaton, AZ: On or about 21 March 2013, and at times prior thereto, the employer did not prepare a written training certificate record to verify that employees were trained in the recognition of fall hazards and the use and operation of guardrail systems, personal fall arrest protection systems, safety net systems,warning line systems,saftey monitoring systems, controlled access zones, or any other protective systems, or the limitations of equipment during performance of roofing work on low-sloped roofs.
