29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about March 21, 2013 during construction activities for the installation of a water line service vault the employer did not ensure employees working inside a trench approximately 9 feet, 4 inches wide, approximately 36 feet long, with a depth of approximately 6 feet at the north end sloping to approximately 8 feet at the south end, had a stairway, ladder, ramp, or other safe means of egress located in the trench.
