29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about August 21, 2015, during the installation of a waterline, the employer did not ensure employees working inside a trench, with a varying depth of approximately 5.2 to 6 feet and near 90 degree vertical walls, were protected from cave-ins by an adequately designed protection system. The soil was classified type B .
