29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    The employer does not train employees in the purpose and function of the energy control program. This violation occurred on or about March 21, 2013, in the production area where employees exposed to a struck-by hazard when they operated, set-up, unjammed, serviced and/or performed maintenance on industrial machinery without being trained in the control of hazardous energy for the machinery.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that adequate training is provided to ensure that the purpose and function of the energy control program is understood by employees.

This violation occurred on or about March 21, 2013 on Line #17 where an employee was exposed to a struck-by hazard while setting up a 60 psi pneumatic cutter without the air being disconnected.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that machines or equipment are turned off or shut down using the procedures established for the machine or equipment.

This violation occurred on or about March 21, 2013, on Line #17 where an employee was exposed to a struck-by hazard while setting up a 60 psi pneumatic cutter without the guillotine blade being guarded.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that points of operation of machinery are guarded to prevent employees from having any part of their body in the danger zone during operation cycles.

The employer does not ensure that unused openings in cabinets, boxes, and fittings are effectively closed.     a. This violation was observed on or about March 26, 2013, at or near Line #5 where employees were exposed to an electrical shock hazard while operating machinery with two open holes on the front cover of a 110 VAC electrical panel.     b. This violation was observed on or about March 25, 2013, at or near Line #8 where employees were exposed to an electrical shock hazard while operating machinery with one open hole on the side cover of a 110 VAC electrical panel.     c. This violation was observed on or about March 25, 2013, at or near Line #12 where employees were exposed to an electrical shock hazard while operating machinery with one open hole on the lower side of a 110 VAC electrical cabinet.     d. This violation was observed on or about March 25, 2013, at or near Line #16 where employees were exposed to an electrical shock hazard while operating machinery with one open hole on the top cover of a 110 VAC electrical junction box.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that unused openings in cabinets, boxes, and fittings are effectively closed.

This violation was observed on or about March 25, 2013, at or near Line #2 where employees were exposed to an electrical hazard while operating a Davis-Standard extruder machine that had an electrical cord with approximately 5 separate splices held together by wire nuts.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that flexible cords are used in continuous lengths without splice or tap.
