29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: a) On or about March 25, 2013, the employer failed to ensure that employees who were provided filtering face pieces for optional use were provided with Appendix D of the respiratory protection standard. Employees were not informed of the potential hazards associated with wearing a respirator. The employer provides 3M 8211 N95 Dust Masks.
