29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     On AR about 19 Mar 2013 at the job site located at 3227 Robbins Road, Alexander AR, the employer did not ensure employees exposed to falling object hazards wore hard hats to protect themselves.  As a result employees working on the ground below employees working approximately 11 feet 3 inches on the upper level were exposed to falling objects that could strike employees on the head.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)  NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE.     On or about 19 March 2013, at the job site located at 3227 Robbins Road in Alexander AR the employer did not ensure employees setting floor joists were protected by a fall protection system.  This condition exposed employees to a fall of approximately 11 feet 3 inches to the unfinished ground below.

29 CFR 1926.1053(a)(8): A metal spreader or locking device was not provided on each stepladder to hold the front and back sections in an open position when the ladder was being used:      On or about 19 Mar 2013 at the job site located at 3227 Robbins Road, Alexander AR, the employer did not ensure employees used a ladder as designed.  The employee used an 8 foot "A" frame step ladder to apply moisture barrier wrapping to the external walls of the dwelling.  The employees used the "A" frame ladder folded and leaned against the walls of the home without engaging the metal spreader or locking device.  This conditions exposed the employees to fall hazards.
