29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: A. Frank Wallace worksite located at 9790 Clark Dr., Rossford, Ohio: On or about March 26, 2013, the employer did not ensure employees wore proper eye protection while operating pneumatic nail guns to secure asphalt shingles. Two employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

Frank Wallace worksite located at 9790 Clark Dr., Rossford, Ohio: On or about March 26, 2013, the employer did not ensure employees were protected from falls while performing weatherproofing activities on the low sloped, three story commercial structure. Employees were exposed to a fall distance of approximately 30.00 feet while entering and exiting the dormer window without fall protection.
