29 CFR 1926.20(b)(2): Programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  A. Frank Wallace worksite located at 4645 Lewis Ave., Toledo, Ohio: On or about June 10, 2015, the employer did not ensure frequent and regular jobsite inspections were conducted to identify and recognize hazards including but not limited to the lack of personal protective equipment and fall protection.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  A. Frank Wallace worksite located at 4645 Lewis Ave., Toledo, Ohio: On or about June 10, 2015, the employer did not ensure employees wore proper eye protection while operating pneumatic nail guns to secure asphalt shingles. Three employees were exposed to potential flying object hazards while operating the pneumatic fasteners.   Frank Wallace was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.95, which was contained in OSHA inspection number 897933, citation number 01, item number 01 and was affirmed as a final order on June 15, 2013, with respect to a workplace located at Rossford, Ohio.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.   A. Frank Wallace worksite located at 4645 Lewis Ave., Toledo, Ohio: On or about June 10, 2015, the employer did not ensure employees were protected from falls while applying weatherproofing material on the single story commercial structure. Employees were exposed to a fall distance of approximately 9.00 feet from the 6/12 pitched roof.   Frank Wallace was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.95, which was contained in OSHA inspection number 897933, citation number 01, item number 02 and was affirmed as a final order on June 15, 2013, with respect to a workplace located at Rossford, Ohio.
