29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Construction Site - On or about March 13, 2013 employees tying rebar on shored forms were not protected from fall hazard of 9 feet to ground. There were no guard rails on the open sides of the shored forms and employees were not using personal fall arrest systems or safety net system.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: (a) Construction Site - On or about March 13, 2013 vertically protruding rebar were not guarded. None of the multiple vertically protruding rebar throughout the project were guarded.
