29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a. On the 2nd floor west side corridor, a ground prong was missing from a three way extension cord that employees were using to power tools in the courtyard, thereby exposing employees to a contact hazard. b. On the 2nd floor courtyard, a ground prong was missing from the Bosch radio/battery charger where a miter saw was plugged into, thereby exposing employees to a contact hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. On the 5th floor hallway where employees were cutting lumber, the strain relief for the plug on the circular saw was pulled away from the insulation of the cord, thereby exposing the employees to a contact hazard.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. In the following areas of the worksite where employees were framing the interior walls of the apartments, holes were cut into the floor and left uncovered, thereby exposing employees to fall hazard. a.
