OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by of falling load hazards:    Frequent and periodic inspections were not performed on the 1-Ton Dayton Trolley hoist.  Employees were exposed to struck-by hazards associated with falling loads and/or an overloaded hoist.    Among other methods, one feasible and acceptable abatement method is:    Perform frequent and periodic inspections as required by ASME B30.16-2007 - OVERHEAD HOISTS, Section 16-2.1.2 and 16-2.1.3.  Inspections include, but are not limited to, controls and operating mechanisms for improper operations; safety devices for malfunction; deterioration or leakage in air systems; and, deformations, chemical damage, or cracks in hook.

29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Employees performing servicing and maintenance operations were exposed to hazards associated with the unexpected energizing or start-up of the equipment such as but not limited to the Mattison Surface Grinder machine; the Butler Newall Grinder machine; the Acer E-Mill machines and the Trinco Dry Blast machine.  Written machine specific energy control procedures for these machines were not developed and implemented.    b) The current written machine specific energy control procedures for all the CNC machines did not include clear, specific and detail with all the steps by steps procedural.    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.179(j)(2):     The following items were not be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections:    Frequent inspections - (Daily to monthly intervals) were not performed for the 5-Ton DEMAG crane, 2 of the 10-Ton DEMAG cranes, and the 10-Ton KONE cranes to ensure the integrity of the cranes for the following items such as but not limited to:  All functional operating mechanisms; Deterioration or leakage in lines, tanks, valves, drain pumps, and other parts of air or hydraulic systems; Hooks; Hoist chains; All functional operating mechanisms for excessive wear of components; and Rope reeving for noncompliance with manufacturer's recommendations.  Employees were exposed to struck-by hazards associated with falling loads and/or an overloaded cranes.    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.184(d):     Sling(s) and fastenings were not inspected each day or during use and were not removed from service when damaged or defective:    Employees were exposed to struck-by of falling load hazards in that one of the yellow nylon slings of the 1-Ton Dayton Trolley hoist was damaged and was not taken out of service.    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.212(a)(4):     Revolving drum(s), barrel(s), or container(s) were not guarded by enclosure(s) which were interlocked with the drive mechanism so that the barrel(s), drum(s), or container(s) could not revolve unless the enclosure(s) were in place:    Machine Shop area in Building 6A - Employees were exposed to laceration, caught-by, flying objects and possible amputation injuries in that the following computer-numerical control (CNC) machines did not have functional door interlocks.  The safety interlocks have been by-passed:    a) Yeong Chin CNC Mill - YCM 1 TV Series 146B  b)  Daewoo CNC Mill    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
