29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: Location: 1419 Carroll Street, St. Louis, MO 63104 Three employees were not wearing hard hats as they were observed walking underneath a load of wall boards with two wall board carts placed on top. The employees walked underneath the load to enter the building at the northwest corner.

29 CFR 1910.180(h)(3)(vi): The operator failed to avoid carrying loads over people: Location: 1419 Carroll Street, St. Louis, MO 63104 The operator of the truck mounted Hiab XS 335 K wall board crane carried a load of wall boards and two carts directly over workers when off loading the building materials at the northwest corner of the job site.
