OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a. Employees operating the line 6 Brampton Engineering winder machine were exposed to hazards such as in-running nip points on the winder rollers which may cause finger, hand, or arm crushing injuries or amputations. The emergency stop controls had shrouds/guards installed around the buttons.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: a. On production line 1, the winder machine is not adequately guarded, exposing employees to rotating parts and in-running nip points during the hand cut mode and product measuring operations. b. On production line 2, adequate machine guarding was not provided on the winder machine to protect employees from rotating parts and in-running nip points during the hand cut mode and product measuring operations. c. On production line 3, adequate machine guarding was not provided on the winder machine to protect employees from rotating parts and in-running nip points during product measuring operations.

29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective: a. In the production area between winders 1 and 2, the flexible cord on an Air King high velocity air circulating fan was not properly grounded. Optimum Plastics, LLC was previously cited for a violation of this occupational safety and health standard (29 CFR 1910.304(g)(5)), which was contained in OSHA inspection number 313152969, citation number 1, item number 3 and was affirmed as a final order on January 25, 2010, with respect to a workplace located at 1188 S. Houk Road, Delaware, Ohio 43015.
