29 CFR 1910.136(a): General requirements. The employer shall ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards. a. At the workplace, the employer did not require the use of protective footwear for employees that were exposed to foot injuries from falling drop out shafts. On September 6, 2013 a drop out shaft fell on an employee's foot resulting in broken and crushed toes.

At the workplace, the employer did not record an injury that occured on September 6, 2013, when a drop out shaft fell on an employees foot.
