29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about 28 March 2013 at the job site located at 2413 Lakewood Circle, Cabot Arkansas the employer did not ensure employees wore protective hard hats while working below workers on the upper level of the construction project.  As a result employee were exposed to struck by hazards from dropped tools and materials as other employees worked 12 feet above their heads.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:     On or about 28 March 2013, at the job site located at 2413 Lakewood Circle, Cabot Arkansas, the employer did not ensure electrical equipment was free from recognized hazards.  As a result the employer used a flexible cord that had damage to the outside insulation which exposed employees to electric shock hazards.

29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose.   On or about 28 March 2013 at the job site located at 2413 Lakewood Circle, Cabot Arkansas, the employer did not ensure the job- made electrical outlet was approved for the purpose it was being used for.  As a result employee(s) were exposed to electric shock hazards while using the circular saw, nail gun and reciproicating saw on the job site.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On are about 28 March 2013 at the job site located at 2413 Lakewood Circle, Cabot Arkansas the employer did not ensure fall protection was provided for employees exposed to a fall of at least 10 feet to the unfinished ground below.  The employer did not attempt to use any form of fall protection.  Employees were engaged in stick framing for a new home construction project.
