29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about 28 March 2013 at the job site located at 2413 Lakewood Circle, Cabot Arkansas the employer did not ensure employees wore protective hard hats while working below workers on the upper level of the construction project. As a result employee were exposed to struck by hazards from dropped tools and materials as other employees worked 12 feet above their heads.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: On or about 28 March 2013, at the job site located at 2413 Lakewood Circle, Cabot Arkansas, the employer did not ensure electrical equipment was free from recognized hazards. As a result the employer used a flexible cord that had damage to the outside insulation which exposed employees to electric shock hazards.

The employer did not attempt to use any form of fall protection. Employees were engaged in stick framing for a new home construction project.
