29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    (a) West side, new commercial construction located at 1750 Martin Luther King Jr. Blvd., Columbus, GA 31906: Employee worked at heights up to 20-feet on a job-made scaffold (Decade MACX Recycling Bin) which rested on the forks of a Gradall Material Lift (Model 534C-6) without being secured to the forks to prevent accidental movement.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    (a) West side, new commercial construction located at 1750 Martin Luther King Jr. Blvd., Columbus, GA 31906: Employee working at heights of approximately 14-feet on an elevated job-made work platform resting on the forks of a Gradall Material Handler was not provided with fall protection. Employee worked inside a Decade MACX recycling bin which had side walls that measured approximately 31-inches high and was not secured to the forks.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    (a) Steel roof purlins, new commercial construction located at 1750 Martin Luther King Jr. Blvd., Columbus, GA 31906: Two employees installing wall insulation and metal siding panels were using metal roof purlins as a work surface which was  located approximately 20-feet, 6-inches from the concrete surface below without a fall protection system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) West side, new commercial construction located at 1750 Martin Luther King Jr. Blvd., Columbus, GA 31906: The 28' Werner (Model D1528-2) extension ladder provided to access the steel roof purlins was not extended 3' past the working surface or secured to the structure. Employees accessing this ladder form the roof were exposed to falls of approximately 20-feet, 6-inches to the concrete surface.
