29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: Job site: On or about September 25, 2013, and times prior to, employees constructing an industrial metal building were lifted to the building roof on a forklift supported platform. The forklift had posted warnings against riders on it and its attachments. This exposed the employees to the hazards of falling and being struck by an overturning forklift.

29 CFR 1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI 92.2-1969 and this section and to be as safe as the equipment was before the modification: Job site: On or about September 25, 2013, and times prior to, employees used a modified extensible boom aerial lift to lift and place suspended bundles of metal roof panels on a building roof. The employer had not obtained any certification for the modification. This exposed the employees operating and riding in the lift to the hazards of an overturning or failing aerial lift and falling or being struck by the aerial lift.

The long rope lanyards would have allowed a falling employee to free fall more than 6 feet and in some circumstances contact the floor. This exposed the employees to the hazard of falling over 6 feet.
