29 CFR 1910.133(a)(1):     The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    On 4/02/2013, employees working at Kemco Aerospace Manufacturing located at 3616 Scarlet Oak Boulevard, St. Louis MO, were exposed to an eye hazards.  Specifically, an employee using or near the horizontal broach machine, serial #6089, was not wearing safety glasses; therefore, the employees eyes were exposed to chemicals and flying particles.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    On 4/02/2013, employees working at Kemco Aerospace Manufacturing located at 3616 Scarlet Oak Boulevard, St. Louis MO, were exposed to a struck-by hazard.  Specifically, a wood 2x4 had been attached to the hydraulic ram of a horizontal broach machine, serial #6089, to prevent the ram from drifting at the end of the cycle and allowing the broach blade to travel through the fixture.  If the blade is allowed to travel through the fixture, the machine must be locked-out and tagged-out to remove and reinstall the blade.  The 2x4 was held in place with a C clamp and no machine guarding was added to protect the employees in the event of the 2x4 shattering.

29 CFR 1910.305(b)(2)(i):     All pull boxes, junction boxes, and fittings were not provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    On 4/02/2013, employees working at Kemco Aerospace Manufacturing located at 3616 Scarlet Oak Boulevard, St. Louis MO, were exposed to an electrical hazard.  Specifically, the electrical box near the operator's controls on the horizontal broach machine, serial #6089, had two exposed red wires with 120 VAC.
