29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): At 3616 Scarlet Oak Blvd, Saint Louis, MO, 63122, On January 6 and 7, 2026, Employees are exposed to point of operation hazards from an improperly guarded Toyokoki Co. press brake, Model 5613 W Mfg. No. 7001962. An employee was utilizing the press without the hand restraint devices and within reach of the foot actuation, allowing his hand to be crushed in the die area when it accidentally activated.
