29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). Worksite: The aluminum extension ladder that was being erected as part of a ladder jack scaffold and held by an employee came in contact with an energized 13800 volt primary power transmission line. On or about 3/26/2013 an employee received a severe electrical shock when the aluminum extension ladder he was holding came in contact with a bare energized 13800 volt primary transmission line.

29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive side rails. Worksite: The aluminum extension ladder that was being used to support a ladder jack scaffold and that was within close proximity to energized 13800 volt primary power transmission lines were not equipped with non-conductive side rails. On or about 3/26/2013 an employee received a severe electrical shock when the aluminum extension ladder he was holding came in contact with a bare energized 13800 volt primary transmission line.
