29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  On or about March 27, 2013, at 2941 Lawrenceville-Suwanee Road, Suwanee, Georgia:  a)  Three employees performing stucco work from the second buck of a tubular welded frame scaffold were exposed to a fall hazard of approximately 14.5-feet when the work platform was not fully planked.   b) One employee performing stucco work from the first buck of a tubular welded frame scaffold was exposed to a fall hazard of approximately 7.8-feet when the work platform was not fully planked.

29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:  On or about March 27, 2013, at 2941 Lawrenceville-Suwanee Road, Suwanee, Georgia:  a) Four employees performing stucco work from a tubular welded frame scaffold were exposed to fall hazards that ranged from approximately 7.8-feet to 14.5-feet when they crossed over a change of direction that was designed by a qualified person on the west side of the building.   b) Four employees performing stucco work from a tubular welded frame scaffold were exposed to fall hazards that ranged from approximately 7.8-feet to 14.5-feet when they crossed over a change of direction that was not designed by a qualified person on the south side of the building.   c) Four employees performing stucco work from a tubular welded frame scaffold were exposed to fall hazards that ranged from approximately 7.8-feet to 14.5-feet when they crossed over a change of direction that was not designed by a qualified person at the southwest corner of the building.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.   On or about March 27, 2013, at 2941 Lawrenceville-Suwanee Road, Suwanee, Georgia:   An employee performing stucco work was exposed to a fall hazard of approximately 14.8-feet when he climbed the scaffold frame of a tubular welded frame scaffold that did not have a ladder installed on the west side of the building.
