29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: b) On or about March 28, 2013 at the jobsite located at 1220 S. Wood Street, Chicago, IL. - employees performing decking operations on the fifth floor of a building were exposed to fall hazards due to an unprotected elevator shaft, thereby exposing the employees to fall hazards approximately fifty six (56) feet. Quinco Steel, Inc. was previously cited for a violation for this Occupational Safety and Health Standard or its equivalent Standard, 29 CFR 1926.760(a)(1), which was contained in OSHA Inspection Number 315154740, Citation Number 1 Item Number 3 and was affirmed as final order on July 06, 2011 with respect to the workplace located at 3748 W. Ogden Avenue, Chicago, IL 60623. Quinco Steel, Inc. was previously cited for a violation for this Occupational Safety and Health Standard or its equivalent Standard, 29 CFR 1926.760(a)(1), which was contained in OSHA Inspection Number 312594328, Citation Number 2 Item Number 2 and was affirmed as final order on October 13, 2009 with respect to the workplace located at Michigan Avenue at the Bridge over the Chicago River, Chicago, IL 60611.
