29 CFR 1915.159(a)(9): Anchorages were not capable of supporting at least 5,000 pounds (22.2 Kn) per employee attached:  At the Kiewit Olympus TLP (Hull) fabrication project, an employee working on a vessel platform tied off (anchored) to an inadequate and unsubstantial anchorage point using members (7 foot runner) of an incomplete hanging scaffold, while working 160 feet above the lower level (channel water).  Or in the alternative:  29 CFR 1915.159(a)(8): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms:
