29 CFR 1910.169(b)(3)(i): Compressed air receiver(s) were not equipped with indicating pressure gauge(s): At this location: On 05/24/2013, Employees engaged in workplace activities were exposed to struck-by injuries from contact with flying metal shards from Air Receiver suddenly overpressured and not equipped with an indicating pressure gauge. The Air Receiver operated at 50 PSI; was not equipped with an indicating pressure gauge, and was used to support workplace employee sandblasting activities.

29 CFR 1910.169(b)(3)(ii): Valves of any type were placed between the air receiver and its safety valve or valves. At this location: On 05/24/2013, Employees engaged in workplace activities were exposed to struck-by injuries from contact with flying metal shards due to sudden overpressure of the Air Receiver that contained a valve installed between the receiver and its safety valve. The air receiver contained an internal air pressure of 50 PSI with a valve installed in the piping between the air receiver and safety valve.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: At this location: Employees engaged in workplace activities were exposed to electric shock injury from contact with energized cord and plug cable not provided with strain relief protection. The cord and plug cable not provided with strain relief protection; delivered electrical power to portable sandblasting equipment, and operated at 120 volts AC with a height of 42 inches above the ground.
