29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself. (a) Front entry-way roof: On or about March 28, 2013 and at times prior thereto, an employee engaged in waterproofing activites had looped a rope around a support to use for fall protection and in doing so used the snaphook to attched the rope to itself around the beam which could expose the hook to side pressure and/or failure in the event of a fall. Note: Abatement certification IS required for this item.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) Front entry-way roof: On or about March 28, 2013 and at times prior thereto, an employee engaged in waterproofing activites 12.1 feet above the ground was utlilizing a long section robe grab attached to a body harness that would have allowed for the employee to fall greater than 6 feet and possiblly hit the ground prior to the fall protection engaging. Note: Abatement certification IS required for this item.
