29 CFR 1910.179(e)(4): Bridge truck(s) were not equipped with sweep(s) which extended below the top of the rail and projected in the front of the truck wheel: a) Inside the west end of Bay 2, on April 1, 2013, the overhead crane (unknown name, serial number and model number) was missing all of the rail sweeps. This condition exposed employees to struck-by hazards. b) Inside the west end of Bay 3, on April 1, 2013, the overhead Shepard Niles (model number 63656, unknown serial number) was missing all of the rail sweeps. This condition exposed employees to struck-by hazards. c) Inside the east end of Bay 4, on April 1, 2013, the overhead (unkown name, serial number and model number) was missing all of the rail sweeps. This condition exposed employees to struck-by hazards.

29 CFR 1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach: a) Inside Bay 2, on April 1, 2013, a 12 foot bridal set of steel alloy chains did not have an identification tag. This condition exposed employees to struck-by hazards. b) Inside Bay 4, on April 1, 2013, the identification tags for the 10 foot steel alloy chains was not legible. This condition exposed employees to struck-by hazards.
