29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  On or about 04/02/13 at Grovetown, GA: Employees were exposed to fall hazards while performing work at a construction site.  The employer failed to perform frequent and regular inspections of the work site in order to identify and enforce safety standards associated with scaffolding and unguarded wall openings.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or above 04/02/13 at Grovetown, GA: Employees were exposed to fall hazards while performing construction work in the vicinity of an unguarded elevator shaft located on the second floor of the building. The general contractor failed to ensure that a guardrail system was installed and maintained in order to protect employees from falls from a height of 14 feet 6 inches above the ground.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  On or above 04/02/13 at Grovetown, GA: Employees were exposed to fall hazards while performing construction work in the vicinity of three large window panels located on the second floor of the building.  The general contractor failed to ensure that midrails were installed on the window panels in order to protect employees from falls from a height of 14 feet 6 inches above the ground.
