At the end of each calendar year, the employer must:  29 CFR 1904.32(a)(1):  Review the OSHA 300 Log to verify that the entries are complete and accurate, and correct any deficiencies identified;  a.  At the end of 2012, the employer created an OSHA 300A Summary of Work-Related Injuries and Illnesses from the 2012 OSHA 300 Log of Work-Related Injuries and Illnesses.  The total number of other recordable cases on the summary was recorded as "1" but the 2012 OSHA 300 Log had two cases recorded in Column (J), Other Recordable cases. The employer had not reviewed the 2012 300A Summary for accuracy and corrected any deficiencies that were found.
