29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about April 5, 2013 during construction activities including; the installation of a plywood filler pieces on the unprotected sides and edge of a flat roof approximately 16. 4 feet above the ground, the employer did not ensure framing subcontractor employees were protected from falling by a guardrail system, safety net system, and/or personal fall arrest system. A & R BUILDERS, LTD. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 63157, CITATION NUMBER 01, ITEM NUMBER 002, AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 5, 2011 WITH RESPECT TO A WORK PLACE LOCATED AT 3313 MILWOOD LANE, LOT 92, GREEN, OHIO 44232.
