The written lockout/tagout program did not include the specific procedures required for a shipyard locks and tags plus program by not having the following requirements in their program:    a. enforcing compliance with the lock and tags-plus program and procedures;   b. for preparing, shutting down and isolating energy sources for each affected machinery, equipment, or system to be serviced; does not have a procedure to ensure the authorized employee has knowledge of the hazards associated with the release of energy or that the affected employee is notified;  c. applying the lock and tags-plus system; does not have a procedure that ensures that only authorized employees apply a lock and tags plus system;  d. verifying energy isolation; does not have a written procedure that ensures the authorized employee verifies all sources of energy have been isolated for the machinery, equipment or system that is being serviced;  e. testing machinery, equipment, or system; does not have a written procedure to ensure that all tools and materials, and any non-essential employees are clear of the works area; does not have a written restart procedure;  f. removing lockout/tags-plus systems; does not have a procedure to notify affected employees of the lock and tags plus system removal; inspection of the work area to ensure that nonessential items have been removed; or procedures to ensure that the lock or tags plus system is removed by the authorized employee that applied it;  g. starting up of machinery, equipment, or system that is being serviced; does not have a detailed procedure that ensures that tools and materials are clear, and nonessential personnel are removed from the work area prior to restarting machinery, equipment or system, after it has been serviced;  h. applying lockout/tags-plus systems in group servicing operations; does not have a procedure that addresses group lock and tags plus procedures;  i. standardization; does not have a procedure to standardize the lock and tags plus program;  j. incident Investigation; does not have a procedure to investigate each incident associated with release of hazardous energy sources;  k. training; does not have a procedure to ensure authorized employee training; does not have a procedure to ensure employee proficiency of lock and tags plus program;  m. program audit; does not have an authorized employee appointed to conduct annual audits;  This condition exposed employees to the hazard of uncontrolled energy sources.
