Shipyard, M/V Tustemena:  On or about 04 April 2013 and times prior thereto, two aerial lifts, Grove MZ70 #105 and JLG #107 basket operator panel controls were not identifiable.  This condition exposed employees to struck by and fall hazards.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.  Fabrication/Machinist Shop:  On or about 04 April 2013 and times prior thereto, four (4) emergency lights were inoperative.  This condition exposed employees to smoke inhalation and/or fire hazards from delays in exiting the building.   Tool Room:  On or about 04 April 2013 and times prior thereto, three (3) emergency lights were inoperative.  This condition exposed employees to smoke inhalation and/or fire hazards from delays in exiting the building.   Welding Shop:  On or about 04 April 2013 and times prior thereto, three (3) emergency lights were inoperative. M/V Tustemena, Outside hull area:  On or about 04 April 2013 and times prior thereto, one (1) acetylene and five (5) oxygen cylinder tanks were stored with in-use acetylene and oxygen tanks.  This condition exposed employees to a fire hazard.

29 CFR 1915.153(a)(1): The employer did not ensure that employees used appropriate eye or face protection where there were exposures to eye or face hazards caused by flying particles, molten metal, liquid chemicals, acid, caustic liquids, gases, vapors, or potentially injurious light radiation.  M/V Tustemena, Turnstile area:  On or about 04 April 2013 and times prior thereto, an employee performing grinding operations on turnstile deck with a 120 volt Dewalt hand grinder was not wearing a protective face shield.  Employee was wearing prescription eyeglasses while performing grinding operations.  This condition exposed employees to struck by and burn hazards.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.  M/V Tustemena, Turnstile area:  On or about 04 April 2013 and times prior thereto an air nozzle used for cleaning registered approximately 100 psi.  Air nozzle was not equipped with deflector to reduce the pressure of air flow.  This exposed employees to air embolism/laceration hazards if they contacted their skin.    Seward Ships Drydock was previously cited for a for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307503631, Citation 02, Item Number 01, issued 18 September 2008, with respect to a workplace, located at Seward Ships Drydock, Seward, Alaska.

M/V Tustemena, Turnstile area:  On or about 04 April 2013 and times prior thereto,  the SSD #040, Lincoln Electric INVERTEC V350-PRO, working at 158 Amps, 70.0 Volts, had a damaged cord exposing wires approximately 51.5 inches from the stinger.  This condition exposed employees to an electrical shock hazard.    Seward Ships Drydock was previously cited for a for a violation of this Occupational Safety and Health Standard, or its equivalent standard which was contained in OSHA Inspection Number 307503631, Citation 03, Item Number 02, issued 18 September 2008, with respect to a workplace, located at Seward Ships Drydock, Seward, Alaska.

M/V Tustemena, Outside hull area:  On or about 04 April 2013 and times prior thereto, the acetylene cylinder tank, # 046PAL3350A,  regulator 0-400 PSI gauge cover lens missing was missing a cover and the oxygen cylinder tank, #107190X, regulator 0-4000 PSI gauge was damaged.  This condition exposed employees to a fire hazard.
