Oregon Chip terminal, North Bend, Oregon- (a)The guardrail system for the Gear Drive platform was missing the mid-rail, exposing workers to a fall of approximately 8 feet onto a lower walkway and/or a running conveyor.

29 CFR 1917.151(h)(1): Rotating parts, such as gears and pulleys, that are located 7 feet (2.13 m) or less above working surfaces shall be guarded to prevent employee contact with moving parts. Oregon Chip Terminal, North Bend (a) The rotating 3 1/2 inch shaft, 7 feet from the ground and 18 inches from the access ladder, was not guarded.
