29 CFR 1910.30(a)(1): Portable and powered dockboards shall be strong enough to carry the load imposed on them. On or about March 05, 2013, and prior times thereto, dock levelers servicing dock doors LASC #9 and DAL #13 located in the warehouse were not strong enough to carry the load imposed on them. These conditions exposed employees to safety hazards.

29 CFR 1910.176(a): Where mechanical handling equipment is used, aisles and passageways were not kept in good repair, with no obstruction across or in aisles that could create a hazard. On or about March 05, 2013, and times prior thereto, a ramp passageway leading from warehouse to trailer yard by Dock number 1 was not kept in good repair, exposing employees to tripping, struck-by hazards.

29 CFR 1910.178(p)(1): When a powered industrial truck was found to be in need of repair, defective, or in any way unsafe, the truck was not taken out of service until it was restored to safe operating condition. On or about March 05, 2013, and times prior thereto, a power industrial truck P1953 being used was found with radiator cover plate not properly secured. This condition exposed employees to burn hazards.
