Instance 1.  Bridgeport Milling Machine (#14) had no point of operation guarding.                    Instance 2.  Bridgeport Mill Milling Machine (#15) had no point of operation guarding.                    Instance 3.  Do-All Band Saw had an unguarded area above the guide rollers where employees could                                     access the blade.  Employer installed a guard that abated the hazard.                   Instance 4.  Delta 12 Inch Drill Press had no point of operation guarding.
