29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about April 10, 2013, the employer did not ensure fall protection was provided for employees who are required to ride on a crane while conducting a physical inventory. Employees were exposed to falls approximately 6 to 17 feet while conducting the physical inventory.

29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment: On or about April 10, 2013, the employer did not ensure fall protection training was provided for employees who are exposed to fall hazards approximately 6 to 17 feet while conducting the physical inventory. Employees were told that a body belt was available if employees wanted to wear it while on the crane.
