On or about April 8, 2013, employees were not protected from falling objects including but not limited to hand tools, debris, and other small objects when working on the ground, below the ladder jack scaffold.  Falling object protection was not utilized on and/or around the ladder jack scaffold.

On or about April 8, 2013, the strain relief on the DeWalt Skilz Saw was frayed, exposing the inner wires of the flexible cord.

On or about April 8, 2013, the board used as a work platform on a step-ladder scaffold was positioned and laid across the top/highest rungs of the step-ladders.  Personnel were conducting construction work activities from the board/work platform.

On or about April 8, 2013, the 6-foot stepladder used to support the working platform of the stepladder scaffold was not used for the purposed for which it was designed in that, the stepladder was folded and leaned against the wall.  Personnel were working from the stepladder scaffold while the ladder was in the leaning/folded position.
