29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about April 10, 2013 the inspection revealed that employees were potentially exposed to serious injury from a fall to the lower level in the building 255 mechanical/electrical room at the Portland Air National Guard Base.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: (a) On or about April 10, 2013 the inspection revealed a yellow extension cord with damaged outer insulation located outside of building 255 at the Portland Air National Guard Base in Oregon. (b) On or about April 10, 2013 the inspection revealed a damaged flexible cord on the Dewalt D28402 angle grinder being used outside of building 255 at the Portland Air National Guard Base in Oregon.
