29 CFR 1910.23(c)(1): Open-sided floors or platforms 4 feet or more above adjacent floor or ground levels were not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder: (a) Ensign United States Drilling, Inc. at Pronghorn TP4-HNB, Briggsdale, CO 80611: On and before April 11, 2013 the exposing employer did not ensure open-sided floors that were 4 feet or more above ground levels were guarded by a standard railing or the equivalent, in that the V-door opening of the pipe ramp on the drilling rig floor was open when employees were working on the rig floor when the pipe ramp and catwalk were not in use. Employees accessed the area near the V-door opening. This condition exposed employees to a fall hazard of approximately 15 feet.

29 CFR 1910.23(d)(1)(iii): Flight(s) of stairs with 4 or more risers, less than 44 inches wide and having both sides open were not equipped with one standard stair railing on each side: (a) Ensign United States Drilling, Inc. at Pronghorn TP4-HNB, Briggsdale, CO 80611: On and before April 11, 2013 the flight of stairs attached to the side of the pipe ramp on the drilling rig, leading from the ground level to the rig floor, was not equipped with a proper standard handrail on the ramp side in that the stairs were 28 inches wide with approximately 14 risers. This condition exposed employees to a fall hazard of approximately 15 feet down the pipe ramp.
