29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2): a) Warehouse - Noted on 4/8/13 and at times prior thereto, the employer did not prepare a workplace hazard assessment for employees using powered industrial trucks such as cherry pickers or that add water to forklift batteries containing acid and that require the use of personal protective equipment (PPE). The following elements shall be addressed in the workplace hazard assessment: 1. Select and have each affected employee use the types of PPE that will protect them from the hazards identified in the hazard assessment; 2. Communicate the selection decisions to each affected employee and; 3. Select PPE that properly fits each affected employee. *ABATEMENT NOTE: The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.
